Long hospital stays are hard on children and the families who love them. The Sweet Dream Project creates personalized comfort packages that help transform a hospital room into a space that feels like their own, filled with the colors, characters, and cozy touches each child loves. What started as one act of love has grown into a mission: making sure every child we reach knows they are seen, celebrated, and never alone.

The form below is exclusively for caregivers of the care team at our partner hospitals to complete for children ages four (4) to eighteen (18). Once you’ve submitted it, we’ll get to work crafting the patient’s personalized Sweet Dream Portable Duffle (SDPD). Forms must be submitted by the first of every month to meet the monthly deliveries in the second week of each month.

The more detail you provide, the more personal and meaningful the child’s experience can be.
